Prince George’s County Health Connect
The Prince George’s County Health Connect is the Capital South Connector Entity (CE) under the Maryland Health Benefit Exchange (MHBE). The MHBE makes health coverage affordable and more accessible for residents through Maryland Health Connection (MHC), the state’s health insurance marketplace under the Patient Protection and Affordable Care Act of 2010 (ACA).
Prince George’s County Health Connect provides access to education, information, and enrollment assistance to Prince George’s County. A comprehensive program led by agencies and community partners reaches out to residents ages 18-64 years of age and their families to provide services.

Prince George’s County Department of Social Services – (Hyattsville, Landover, Temple Hills) – Local government offices providing intervention services to strengthen families, protect children and vulnerable adults, encourage self- sufficiency and promote personal responsibility for Prince George’s residents in need of social and well-being supports.
Prince George’s County Health Department /– Local government agency committed to protect the public’s health, assure the availability of access to quality health care services, and to promote individual and community responsibility for the prevention of disease and disability.
CASA – 501©3: We are CASA is the state’s foremost Latino and immigrant organization and a national leader in supporting immigrant families. CASA provides a comprehensive range of services focused on economic empowerment, financial independence, and social, linguistic and political integration.
HCD International – An award-winning, health care management and technology consulting firm committed to providing outstanding professional services to both public and private sector clients. HCDI has extensive outreach, enrollment, navigation, retention, and adherence services and experience serving USHHS, CMS, DHMH, PGCHD and PGCDSS.
Mary’s Center – FQHC: Mary’s Center provides health care, family literacy and social services to individuals whose needs too often go unmet by the public and private systems. Mary’s Center uses a holistic, multipronged approach to help each participant access individualized services that set them on the path toward good health, stable families, and economic independence. The Center offers high-quality professional care in a safe and trusting environment to residents from the entire DC metropolitan regions, including individuals from over 100 countries.
Primary Care Coalition of Montgomery County – 501©3: PCC has over a decade of experience working with community organizations and health care providers to serve vulnerable and hard-to-reach communities by successfully developing trusted voices in needed footprints. PCC manages Health Services programs in Prince George’s and Montgomery County.
Sowing Empowerment and Economic Development, Inc. (SEED) – 501©3: SEED is a multi-faceted service provider agency whose programs include the SEED Food Distribution Center, Coat of Many Colors Clothes Closet, EXCEL Academy Public Charter School, EXCEL Academy Freedom School, SEED Summer Bridge Tutoring Program and the SEED Extended Learning Program. SEED is also a HUD approved housing counseling agency. Through academic achievement, community services, housing and economic development, SEED will create environments where all individuals are empowered, children are nurtured, families are strengthened and communities are transformed.
